How The Near Miss Show works

Catch it at the near miss. Every claim has a source, every number is checked, and every mistake is corrected in public. These are the rules we hold ourselves to on YouTube, LinkedIn and this website.

01

Editorial standards

Sources, in this order

  1. Law and regulation
  2. Official investigation reports (for example US CSB and UK HSE)
  3. Standards bodies, paraphrased only, never copied
  4. Peer-reviewed research
  5. Institution guidance (for example IOGP, CCPS, IChemE, Energy Institute, IOSH, NEBOSH)
  6. Company public reports, for company figures only
  7. AZL’s working papers, always labelled as working papers

Social media posts, unsourced infographics and AI answers are never used as a sole source.

The numbers rule

Every number has a source, year, unit and base. Conversions are shown on screen.

Fairness

Accidents are explained as system failures, not personal blame. Companies are named only from public reports, in neutral language. Opinions and recommendations are labelled as such; proposed criteria are proposals.

Review

AZL is the sole reviewer. Every episode passes six checks before it goes out: topic, sources, script, a deep check of every claim against the primary source for comparison and case-study episodes, a final quality check, and AZL’s approval.

Corrections

Errors are verified within 48 hours, corrected in a pinned comment and the description (or by re-upload if serious), and recorded in the public Corrections log.
